Use this form to order names tags for each member
of your family.
If the last name of an individual is different from the "Household" name,
please use an additional form.
Children under the age of 6 will receive an age appropriate name tag with a clip.
Please state the age of any children.
All other name tags use a magnet to prevent damage to clothing.
A Donation of $3.00 per name tag is requested with a maximum of $12.00 per household.
